Digital Healthcare Initiative Delivering Real Impact

A new digital health initiative is enabling clinicians to support vulnerable patients at home while helping to cut hospital admissions, a Jersey charity has said.


Family Nursing & Home Care (FNHC) has introduced a two-year programme, the Graphnet Remote Monitoring Pilot, designed to help patients live safely and independently in their own homes.

Through digital healthcare platform Luscii, patients can submit regular health updates which are reviewed remotely by medical professionals. If the data highlights potential concerns, healthcare staff can step in quickly and arrange home visits when needed.


Kathryn Kelly of FNHC said the technology was already proving valuable.


“We’ve identified clinical issues through the data coming in and have been able to respond immediately,” she said. “It’s already made a difference.”

During the first year, the pilot will focus on supporting people living with frailty, before expanding to include diabetes and wound care in the second year.


The project has received funding from Impact Jersey’s Care Tech Challenge, which allocated up to £2m for innovative technological solutions aimed at improving healthcare.


Dr Chris Edmond, clinical lead for Impact Jersey, said the initiative aimed to help people maintain their independence and quality of life.


“The goal is to enable people to live their best lives for as long as possible,” he said.


He added that remote monitoring could also ease pressure on Jersey’s healthcare services.

“There can sometimes be concern that technology is about cutting costs,” he said. “But in reality, it allows us to deliver care more efficiently and helps people remain at home for longer.”


Dr Edmond emphasised that digital innovation should complement, not replace, face-to-face care.


“We don’t want a future where people are only interacting with AI or chatbots instead of seeing their doctor - that human connection is vital,” he said. “However, AI and digital tools can help us do more and ensure those personal interactions happen where they are needed most.”
